No less than 130 000 accidents strokes occur each year. At the end of the first month, about 20% of patients die while 75% of survivors have permanent sequelae. It is the leading cause of disability acquired from adults, often aged in Western countries. With the increase in life expectancy, this disease will take in the next few years an unprecedented magnitude. To cope, a restructuring of its management is necessary.
The stroke is an abrupt shutdown of the blood flow to the brain. Cerebral vascular accidents (CVAS) are mostly due either to a clot in the artery (is the case three times out of four), hemorrhage, is kind of breach of the walls of the vessel. The ageing of the population suggests an increase in incidence and the burden of this disease for the company.
Reform the Organization of care
For several years, the experts point out the vital interest of the stroke as an extreme emergency, such as myocardial infarction. Only radiological tests to distinguish the type of strokes and thus to choose the appropriate treatment. The treatment of the stroke requires a multidisciplinary support.
It is now known that support in a specialty Neurovascular unit is accompanied by a reduction of approximately 30% of the deaths or the dependencies on non-specialized units.
Reported to the France, these techniques would result in the reduction of approximately 7,000 deaths or dependencies per year. This benefit is immediate and continued another 10 years after the crisis.
“We know that a comprehensive and structured support allows to reduce significantly the number of deaths and dependence. In managing this disease involves the quality of the whole of the care sector. We must reform the care deeply”. It is in these terms that is expressed by the Minister of health Bernard Kouchner during the presentation of the national programme to combat cardiovascular disease.
Restructure the care
The stay in the units specialty Neurovascular are on average shorter than in non-specialized services. Thus, the benefit in terms of reduction of mortality, disability and duration of hospitalization is accompanied by a reduction in costs of care of patients. Thus, the Germany already has close to 80 units of this type, when the France considered that between 10 and 15.
But it is a restructuring of the entire care sector that is involved with the appointment of Prof. Hommel as national co-ordinator expert. The care sector extends alert home rehabilitation through admission, emergency treatment, access to deriving benefit from the…
President of the Association France stroke, Dr. Jacques Boulliat denounced in October 2001 in an interview to Doctissimo inequality of the capacity of the Department to another. Some areas have several rehabilitation centres while others are lacking.
Bernard Kouchner says that “Prof. Marc Hommel Chu Grenoble shall promote and accompany the restructuring of care within the framework of a multiannual programme on offer. In addition, each regional hospital Agency will designate a coordinator to accompany regional medical project of reorganization; All third-generation healthcare organization regional schemes should integrate treating stroke axis”.
